Annual Health Check Questionnaire

We are pleased that more and more people are informed about their Annual Health Check. We are now focussing on letting families know that the Health Action Plan should always be given after the health check and should support the person to understand their health needs for the coming year.

We coproduced a Health Check Questionnaire with other organisations and colleagues from Suffolk and North East Essex ICB. The questionnaire is currently only for people in North East Essex but the other ICBs are looking to make their own now too.

There are two questionnaires available, one for the person to complete and one for the carer, family, supporter to complete.

Please take some time to complete the questionnaire as it will help to improve services and equity for people with learning disability and or autism.
