Learning Disability – Annual Health Check Questionnaire

We shared previously that we had worked with Suffolk and North East Essex NHS to produce an Annual Health Check Questionnaire. We can now share the links with you so that you can support the person you care for to complete the survey about their most recent annual health check. This will help towards making sure these vital check-ups are consistent and useful in reducing the health inequalities that currently exist for people with a learning disability.

This questionnaire is currently only for people living in North East Essex only, however we understand that some other ICBs in Essex are looking to produce this type of questionnaire too. We will share when these become available.
